The Court Order dated September 16, 2025 provides that the fund may not provide direct disbursements of settlement funds to non-class members unless several specific prerequisites are met:

• The transaction be in the form of an outright purchase of the Class member’s claim
• The purchase was completed prior to the exhaustion of any appeals in the case; and
• The third party provided relevant disclosures to the class member (in the form and manner required under the order) regarding potential tax consequences resulting from the sale

In addition, Verita requires the following items to identify the settlement claim in order to process a transfer:

• Claim ID
• NCAA EC ID
• Athlete's name and current information matching the claim submission
• Government-issued photo identification of the Student-Athlete
• Seller and Buyer's signed W9Form

If these prerequisites are met, and the third party wishes to obtain direct disbursements from the fund, please complete this form. The information you provide must match the information in the Bill of Sale and/or other relevant sales transaction agreement(s).
